For the Russian worker working on their own version of the "Demon Core" experiment, the guy received something like 50 Gy, with his hands receiving something like 1800 ±200 Gy (estimated) - mostly from neutron radiation (90% of the total, give or take), so that would mean a quality factor of 10 to 20. This would put his general irradiation at 2-5 Sv (no less than 1 total-body), but his hands recieved upwards of 100 Sv. The guy was pronounced dead (heart failure) about 70 hours after exposure. -> Criticality accident in Sarov, 1997.

Like others have said, level 10 have happend multiple times, And at-least once been survived. Anatoli Bugorski received between 2000-3000 sieverts of radiation after having his head in a particle accelerator accident, he was 36 at the time iirc. He did receive some damages, paralysis of part of his face, hearing-loss of an exposed ear, fatigue and some epilepsy - but overall relatively unharmed, relatively being the word since he was expected to die within days. Anatoli is for the record still alive and 82 years of age. I really recommend Kyle Hill's video on the incident: "What if you put your head in a particle accelerator?" which goes into why he survived.

Level 10 has happened multiple times. Hisashi ouchi received a somewhat close dose to 20 Sv. There was also a case in 1999 where thieves tried to steel 27,000 Curies of Co-60 (or 999 TBq) One of the men died only 30 minutes after exposure. Another one died shortly after. Boris Korchilov also received a dose higher than 20 Sv. (Something closer to 50+) In Los Alamos there was a case of a plutonium criticality where a worker received at least 30 Sv (some reports say upwards of 120 Sv to the upper chest) There are many cases of 20 Sv and above. I have also heard some claims that a chernobyl worker received a dose of 21 Sv but I am not sure of it at the moment. But the point stands. 20 Sv plus is not very rarely seen. Cobalt 60 with even 20 TBq (half a gram) would have you receive this dose in a few minutes even with 1 meter distance. You wouldn't need a nuclear bomb for this. Just half a gram of cobalt 60 would be more then enough.

one movie that portrays radiation poisoning and fallout well is When the wind blows. it's a brilliant cinematic masterpiece but it is genuinely horrifying and perhaps the most disturbing movie i've watched. the author did a great job of showing us what a nuke would actually do to average civillians and reminding us of just how deadly that situation would be. I highly reccomend it to anyone who hasn't watched.

i once got a dose of uncalled for radiation to my hand specifically at my previous work. Was working in ironworks as a maintenance and industrial cleaning crew. There was one part of the catwalk going above a sheet metal rolling mill that was off limits whenever a control light is on (usually during active work of the rolling mill below turning red hot iron slabs into sheet metal), due to radiation emitted by some part (dunno why it was there even, some kind of weird shielding). The line was off for maintenance and got told to clean up the catwalk. Got my bucket with solvent, a rug and a pressure washer. Done with most parts of the catwalk and proceeded with the shielding part that went paralell to the catwalk. I had to reach over the other side of the shielding with my hand to swipe it clean with a solvant. It was supposed to be off, and was clearly off when i got up there as marked by the green light. As i was going down after i was done (about half an hour of scraping and wiping), i've noticed red light is on. Follow up evaluation deducted that there was a fuckup and some sort of test of said shielding was deducted while i was up there, it was a weird electric powered wall that i have no idea what it actually did, made no noise whatsoever but the side effect of it being on was some kind or radiation being emitted, i think x-rays but not sure here. Deduction was that i've taken a dose (can't remember a specific number now but something around 3 mSv for about 20 minutes of exposure. Over all i've been sent off and told to come for a follow up in case i feel any side effects. Mostly my right hand got exposed and the only effect that i noticed was that my finger nails quickly darkened a little bit in the following two days to the point of being kinda orange/brownish, like they got tanned, and then over the course of the next few days lightened back up and a very thin layer on the top of kinda shed/peeled (like snake skin). Never had any issues with that hand since then, no other side effects, but if i remember correctly there was some safety issue drama revolving around that incident and the dude who conducted the test got into minor trouble and a big talk with the supervisors. It was around 8 or 9 years ago, and i've even thought about that being just a side effect of the solvant rather than radiation but i've rejected that as an option because we've used that solvant daily with my hands being soaked in it up to elbows sometimes for hours and never had any similar discolouration happen, so i guess i've had my fingernails (or more like hand) irradiated. I'll need some explaination here: are fingernails/ ceratin affected by radiation more or what? Is discolouration of nails normal for that kind of exposure? Is ANY discolouration normal? Maybe was that a resultant sum of the solvant getting hit by radiation and it's that what made the discolouration and not just my nails on their own?
